Warnings About Ketoacidosis
Invokana was approved by the U.S. Food and Drug Administration (FDA) in August 2014 for the treatment of diabetes.
The medication is a glucose co-transporter 2 (SGLT2) inhibitor that increases sugar removal in the kidneys, reduces the amount of sugar produced in the liver and limits the amount of sugar absorbed through the stomach and intestines.
However, shortly after the drug was approved, the FDA received 20 reports of SGLT2 inhibitors causing ketoacidosis. In each case, victims were taken to the emergency room.
This led to the FDA issuing a drug safety alert in May 2015 noting that SGLT2 inhibitors could lead to ketoacidosis. The SGLT2 inhibitors named in the alert included:
- Invokana
- Invokamet
- Farxiga
- Xigduo XR
- Jardiance
- Glyxambi
The alert advised patients to immediately seek medical attention if they experience symptoms of ketoacidosis, including:
- Pain in the abdominal area
- Nausea
- Vomiting
- Confusion
- Fatigue
- Drowsiness
If doctors confirm the presence of ketoacidosis, they should take patients off SGLT2 inhibitors and take appropriate steps to treat the condition.
The FDA updated this drug safety alert in December 2015, stating it was adding new Warnings and Precautions to SGLT2 labels informing patients of the risks of ketoacidosis.
A review of the FDA’s Adverse Event Reporting System (FAERS) found 73 cases of ketoacidosis in patients taking an SGLT2 inhibitor to treat diabetes between March 2013 and May 2015.
Research on Invokana and Ketoacidosis
In 2015, the University of San Diego conducted an 18-week study of the safety and effectiveness of Invokana in 351 patients with type 1 diabetes.
Researchers randomly gave patients 100 mg or 300 mg doses of Invokana or a placebo. Five patients who took 100 mg doses of Invokana and seven who took 300 mg doses of Invokana suffered ketoacidosis, while none of the patients on a placebo suffered ketoacidosis.
Researchers concluded that lower doses of Invokana may be necessary to avoid ketoacidosis.

Other Side Effects of Invokana
The FDA’s updated drug safety alert in December 2015 also warned about the risk of serious urinary tract infections, some of which could lead to kidney infections.
The alert warns people taking Invokana to watch for signs of a urinary tract infection, including:
- Burning sensation when urinating
- Frequent urination
- Feeling the need to urinate right away
- Fever
- Pain in the lower stomach
- Blood in the urine
Patients who experience any of these symptoms should consult a doctor to find out if it is a urinary tract infection.
In June 2016, the FDA announced it was strengthening its warning about the risk of severe kidney injury while taking SGLT2 inhibitors like Invokana.
The FDA revealed it had received 101 reports of acute kidney injury in patients taking these medications, with some people being hospitalized and put on dialysis.
Symptoms of a kidney injury include swelling in the feet or legs and decreased urine. Anyone who experiences these symptoms should seek treatment. However, patients should not stop taking it unless their doctor says so, as this can cause blood sugar levels to get out of control.
